What are lactic and glycolic acids?

Lactic and glycolic acids are naturally occurring alpha hydroxyl acids (AHA’s) found in fruits and other foods.  AHAs are used in formulation to exfoliate or shed the surface skin.  The extent of exfoliation depends on the type and concentration of the AHA.  At high concentration, AHAs function as peeling agents, which act more rapidly, and at a deeper level.

Tummy Tuck/Abdominoplasty

Known commonly as the “tummy tuck”, abdominoplasty is a surgical procedure to remove excess skin and fat from the abdomen.  The procedure also helps to flatten the abdomen by tightening the surrounding muscles.  If you are in relatively good shape but are concerned by fatty deposits or loose abdominal skin that won’t respond to diet or exercise, this procedure can dramatically reduce or even eliminate a protruding abdomen.  Abdominoplasty can be particularly helpful to women with stretched abdominal areas due to previous pregnancies.  Older individuals whose skin has lost some of its elasticity and are slightly obese can also benefit from a tummy tuck. 

During the procedure, the skin and fatty deposits on the abdomen are removed.  The weak, bulging abdominal muscles are tightened to flatten and strengthen the abdomen.  The excess skin is redistributed to cover the entire abdomen, resulting in a tighter, flattened abdomen.  The surgery can produce a permanent scar, although the scar will likely be located below the bikini or bathing suit line and will fade over time.

Eyelid Surgery/Blepharoplasty

Eyelid surgery, or blepharoplasty, is a procedure to remove excess skin and muscle from the upper and lower eyelids.  Bags under eyes, drooping eyebrows, and loose wrinkled skin can give you a tired, sad or worn down appearance to your entire face.  Blepharoplasty is a common procedure that can be combined with other cosmetic procedure to enhance the appearance of the face, forehead, chin, cheekbones, etc., and create a rested, more youthful appearance.  This is one of the most common procedures requested by men. 

Eyelid surgery is performed by making incisions within the natural creases of the upper and lower eyelids.  The lower eyelid incision can often be made on the inside surface to avoid a scar on the skin surface.  The excess skin and any bulging fat is removed from selected areas.

Botox

Botulinum injections, also referred to as Botox injections are a temporary non-surgical procedure to relax wrinkles around the eyes and forehead.  This commonly performed and popular minimally invasive procedure uses a FDA approved medication delivered via localized, pinpoint injections.  Patients notice a clear improvement in these wrinkles usually within 3 days.

This procedure is ideal for the patient with minimal wrinkles and crows feet with virtually no recovery time.  The wrinkles that respond best to Botox treatment are those that are caused by muscles, such as from frowning or squinting.  The effect lasts an average of four months.

Breast Augmentation

Breast augmentation, or augmentation mammaplasty, is a surgical procedure to enhance the size and shape of a woman’s breast.  This commonly performed procedure must be carefully tailored to each woman, to match both the woman’s preference for size and shape, as well as the stature and size of the patient. 

During your initial consultation, Dr. Boland will evaluate your health and determine which surgical techniques are most appropriate for you.  If your breasts are sagging, Dr. Boland may also discuss a breast lift.

During the procedure, an implant is inserted through an incision made in the crease under the breast, around the areola (dark tissue around the nipple), or through the armpit.  The implant may be placed under the breast tissue or beneath the chest wall muscle.  Dr. Boland takes special care to assure that the incision scar will be as subtle and unnoticed as possible.  Whether your goal is to correct developmentally misshapen breasts, to enlarge the breast size, to alter the appearance after pregnancy or age, or to enhance one’s self appearance, we can assist you in obtaining the desired result.

Breast Reduction

Breast reduction surgery, or reduction mammaplasty, is designed to reduce the size of the breasts and create a more attractive breast shape and form.  Large breasts can cause a woman breast pain; back, shoulder and neck pain; posture problems; shoulder grooves from bra straps; and skin rashes under the breasts.  Women with extremely large breasts often also suffer emotional embarrassment and are limited in a number of physical activities.  The goal is to make the breast size more proportional to the rest of the body, and relieve physical and emotional discomforts. 

Breast reduction surgery removes excess skin, fat, and glandular tissue, resulting in smaller, lighter and firmer breasts that are better-shaped in proportion to the rest of the body.  Predefined incisions are made on the breast, and excess skin, fat and glandular tissue are removed.  The nipple is then elevated to its new position on the breast mound, and the remaining tissues are closed around it.  Some liposuction of adjacent areas may be performed at the same time to improve the shape of the new breasts.  While breast reduction surgery is often covered, liposuction to contour the breasts is not.

Breast Reconstruction

Mastectomy is a traumatic experience for any woman.  Breast reconstruction is an alternative for women of any age providing your health is good enough for you to tolerate a general anesthetic. 

There are two basic ways of reconstructing a breast or both breasts; prosthetic implants or using you own tissue.  Your surgeon will discuss with you the risks and benefits of implants versus using your own tissue to reconstruct your breast(s).

Prosthetic implants usually consist of a silicone shell enclosing saline (salt water), or silicone.  The implants are usually placed underneath the muscle of the chest wall.  Although implant surgery is usually simpler and shorter than using you own tissue, the complication rate of implant surgery may be higher if you have had radiation to the chest skin.

After mastectomy, the skin envelope may not be large enough to allow for an implant and skin expansion may be required before the “permanent” implant is inserted.  Tissue expansion is done by inserting an expandable silicone “balloon” which is periodically inflated with saline.  When enough skin is available, the expander is removed and an implant can be inserted. 

Using your own tissue is called autologous reconstruction.  Autologous reconstruction involves transferring soft tissues (skin & fat) from other areas of the body.  Because of frequent excess tissues and because of the convenient anatomy, skin from the lower belly is often a preferred area for harvesting autologous tissues for breast reconstruction (TRAM flap reconstruction).  Less commonly, other areas can also be harvested, i.e. buttock, thigh or back.  Complications of using your own tissue include the chance that the transplanted tissue does not survive and a hernia if the tissue is taken from the abdomen.

Most women can get back to work 2-4 weeks after implant surgery, although repeated visits to the doctor for soft tissue expansion may be required along with the second operation to insert a permanent implant.  After autologous tissue reconstruction, most women are back to work by one to two months.  It is important to understand that a “normal” breast will never be obtained with a breast reconstruction operation, and realistic expectations are in order.  However, patient satisfaction is generally high.

Liposuction

Liposuction, also called lipoplasty or suction assisted lipectomy, removes deposits of excess fat from specific areas of your body.  For example, it can slim your hips and thighs, flatten your abdomen, reshape your waist, or eliminate a double chin. 

Liposuction is particularly well suited for men and women who have relatively normal weight but isolated areas of fat that cause their body to appear disproportionate.  These localized fat deposits may sometimes be an inherited trait and typically do not respond to diet and exercise.  Liposuction is often the only way to eliminate them.

Through a tiny incision, a narrow tube or cannula is inserted and used to vacuum the fat layer that lies deep beneath the skin.  The cannula is pushed then pulled through the fat layer, breaking up the fat cells and suctioning them out.  There are several liposuction techniques that can be used to improve the ease of the procedure and to enhance outcome.  These include ultrasound-assisted lipoplasty (UAL), the tumescent technique, and the super-wet technique.

Breast Lift/Mastopexy

A breast lift, or mastopexy, is s surgical procedure to raise and reshape sagging breasts.  Many women seek mastopexy because pregnancy and nursing have left them with stretched skin and less volume in their breasts.  The procedure can also reduce the size of the areola (the darker skin around the nipple).  This operation can be combined with placement of a breast implant to not only improve the shape and contour of the breast, but also increase the size. 

In mastopexy, the excess skin is removed from the lower part of the breast, and the nipple and colored areola are moved along with the entire breast mound to a higher position.  Incisions are usually made around the areola, then carried vertically downward and occasionally within the crease under the breast.  The goal is to elevate the position of the nipples, tighten and lift the breast tissue, and create a more youthful appearing beast shape.  Some patients, especially those with relatively small breasts and minimal sagging, may be candidates for modified procedures requiring less extensive incisions.

Ear Reshaping/Otoplasty

Ear Reshaping, or Otoplasty, can change the shape of your ears to improve their appearance, and put them in balance with the rest of your face.  It is commonly performed to correct protruding or excessively large ears.  Surgery can be performed at any age, and is frequently performed on adults.  Children should wait until the ear has reached near full development at around age 5 or 6.  By correcting the ears before a child enters school, potential psychological trauma can be avoided.  This procedure is often covered by insurance. 

Otoplasty involves changing the framework of the ear cartilage, which modifies the way the skin drapes over it, and the overall appearance of the ear.  Otoplasty is usually performed through a small incision hidden on the back of the ear, near the crease.  Once the incision is made, the cartilage is reshaped and the ears set back closer to the head to create the desired look.  The scar is normally inconspicuous and well-hidden after it has healed. 

Rhinoplasty/Nose Reshaping

Rhinoplasty is a procedure that sculptures the nose, changing its shape and /or size.  It can be performed to meet cosmetic goals or for reconstructive purposes such as birth defects or breathing problems.  Small changes in the nose can often have a significant, noticeable impact on your overall appearance.  Therefore our staff will work closely with you to learn how you would like your nose to look, to evaluate the structure of your nose and face, and to explain the factors that can influence the procedure and the results. 

There are two basic approaches for rhinoplasty-closed and open.  The closed technique involves placing all the incisions inside the nose.  The open approach includes incisions inside the nose as well as a small incision between the nostrils.  This external incision usually heals well and is fairly inconspicuous.  The open approach allows for more predictability, flexibility and surgical precision.  It is recommended when several significant changes need to be made.  This surgery is frequently combined with the placement of a chin implant to create more facial harmony.  It can also be combined with surgery to address functional breathing problems such as those related to a deviated nasal septum.

Spider Veins

Spider Veins often develop in the face and legs of many people as they age.  Regardless of their location, spider veins are usually difficult to camouflage with make-up or stockings, and for that reason many seek treatment. 

Treatment for spider veins has evolved in the recent past, now focusing on two methods:  lasers and sclerotherapy. The method most appropriate for your circumstances depends on a variety of factors, including the size and location of the veins.  Whether treated with the laser or sclerotherapy, effective treatment for spider veins is now performed on a daily basis with a negligible recovery period. 

As laser technology has improved, spider veins of the face can often be treated with minimal or no bruising at all.  The underlying spider veins are effectively removed in one or two brief sessions while the overlying skin is left undamaged, avoiding the need for any recovery time.  In most cases, people are able to return to work or their usual activities immediately following the treatment. 

Larger veins of the legs are treated by sclerotherapy.  This rather simple procedure involves injecting a solution into the spider veins using a very fine gauge needle.  This solution causes the veins to collapse and fade.  In addition to the cosmetic benefit, sclerotherapy may also remedy the uncomfortable symptoms associated with spider veins, including aching, burning and night cramps.

The term gynecomastia comes from the Greek words gyne meaning "woman" and mastos meaning "breast." In practical terms, this means abnormally large breasts on men. The most common cause is puberty. Boys often develop at least some excess breast tissue when they reach puberty. The condition usually disappears within a few months or years, but for others the condition remains. Some of the other potential causes are steroid use, genetic disorders, side effects of medications, and aging. Surgery is the only known physical remedy.

Initial recovery time is from 7 to 10 days and complete recovery time is from 4 to 6 weeks. During this time there should be no heavy lifting or any other strenuous activity.
